Smoking Cessation Program
Who We Are
The Barrie & Community Family Health Team's Smoking Cessation program was initiated in 2009. The goal of the program is to decrease the number of patients who smoke within the Barrie & Community Family Health Team patient population. This health prevention program is available to all Family Physicians within the Barrie & Community Family Health Team and is a community collaborative program.
Smoking Cessation Program Overview
The Smoking Cessation program offers:
- An interdisciplinary team approach to supporting patients in smoking cessation
- Partnerships with community agencies to further support patient care needs
- Patient education and self-management resources
Appointment Information
What you can expect for support when you visit your practitioner:
- Ongoing assessment of your smoking status
- Ongoing support to assist with preparing to quit smoking
- Support with developing a comprehensive personal quit plan when you are ready
- Ongoing support once you quit to prevent relapse
- Available support with a dietitian to discuss healthy eating options
- Available support with a pharmacist to discuss medication options
